Nurse Practitioner – 1 FTE and 1 short term contract available
The Central Hastings Family Health Team is currently seeking a full time Primary Health Care Nurse Practitioner – 1 FTE – permanent position
The Central Hastings Family Health Team is a group of collaborative health care professionals, located in the heart of Hastings County. We operate out of offices in Madoc, Marmora and Gilmour. Nurse practitioners on our team work with a variety of other health care practitioners including family physicians, other nurse practitioners, a registered dietitian, social workers, registered nurses and registered practical nurse as well as administrative support staff.
Position Description
This is a full-time position with a wealth of opportunity for the ideal candidate. You will be driven to create, both collaboratively and individually, health care programs that will significantly benefit our population. The successful candidate will implement such programs, as well as assisting with existing programs. Programming will take place in tandem with a satisfying primary care practice in all three of our health care sites.
Main Responsibilities
- Provide primary health care to patients including health promotion, disease prevention and chronic disease management
- Develop and conduct a broad range of health promotion and chronic disease management programs and patient education
- Collaborate with other multi-disciplinary team members and external partners on programming development and day to day issues
- Provide episodic care to patients of the Family Health Team presenting with acute illnesses
- Provide input for the development of the FHT’s program planning
Qualifications and Experience
- Current registration with the College of Nurses in Ontario in the Extended Class RN(EC)
- New Graduates are welcome to apply
- Excellent health assessment skills, knowledge and proficiency in evidence-based methods
- Excellent written and oral communication skills
- Be proficient in EMR use and documentation – Telus PS experience an asset but not a requirement.
- Ability to work collaboratively as a member of FHT

Administration Office & Receptionist Support
The Central Hastings Family Health Team, a non-profit corporation providing primary health care is recruiting a versatile individual to support both the Administration Office and Clinical Reception role to join their Team.
This is a temporary .5 FTE part-time position (40 hours per bi-weekly period) located at the Madoc site requiring occasional travel. For the successful applicant it will also evolve into a casual position.
We are seeking an organized, energetic individual with the ability to work well independently and within the Team framework. The successful applicant will have solid computer skills and attention to detail to provide support. This position is responsible for a wide range of administration such as word processing, spreadsheet, meeting preparation & minutes, web-based technology and medical reception.
The successful applicant will have the above base skills to apply to the administrative support role. Proficiency in MS Office applications, Electronic Medical Records with strong communication skills and an ability to coordinate and prioritize a variety of functions are assets you display.
